We encourage all new patients to complete their required forms prior to their visit. These can be filled out using a desktop or mobile device. If you’re unable to complete them ahead of time, please arrive 15 minutes early so we can assist you in the office.

Appointments
We are currently accepting new patients. You can schedule an appointment online or by calling (904) 448-8688. Please provide at least 48 hours’ notice if you need to cancel or reschedule.
Insurance
We accept most major dental insurance plans and will file claims to help maximize your benefits.
Payment
Payment is expected at the time of service unless prior arrangements have been made. We accept cash, Visa, MasterCard, Discover, American Express, Google Pay, Apple Pay, and CareCredit.
